Cost: $32

There is a drop down menu on the form that says “Provide Card Information.” Click that and to expand it and provide the following information:

1. Name on Card
2. Current Mailing Address (if different from PayPal address)
3. Address when you took class (if different from PayPal address)
4. Approximate Date of Class
5. Name of Instructor
6. Level of Class (Basic, Advanced, Nitrox, Master, etc.)
7. Certification Agency (NAUI, PADI, SSI, or YMCA)
8. Address you want card mailed to (if different from PayPal address)
